What you need to prove your identity and eligibility.
Prepare Before You Apply
The National Verifier requires proof that you are who you say you are, and that you qualify for the benefit. Having clear photos or scans of these documents ready will speed up your approval.
Proof of Identity
You must provide one of the following unexpired documents:
- Driver's license (state, military, or foreign)
- Birth certificate
- Passport
- Government-issued ID (state or tribal)
- Permanent Resident Card
Proof of Income
If applying based on income, provide:
- Prior year’s state, federal, or tribal tax return
- Current income statement (pay stub)
- Social Security statement of benefits
- Veterans Administration statement of benefits
- Unemployment statement
Proof of Address
If your ID has an old address, you may need to provide a utility bill, lease, or mortgage statement to prove where you currently live.
Tip: Never send original documents. Always submit copies or clear photos.
